INSURANCE - INCIDENT REPORT

WestCycle Ride Secure Members are covered by both Public Liability and Personal Accident Insurance. The WestCycle Inc. Member Accident Program is underwritten through HDI Global Insurance and is generally a reimbursement policy, meaning the member must pay for the treatment to then be reimbursed. You may submit an incident report using the form below. Please note that this is not a claim form, meaning you should provide general details only. Once this form is submitted and reviewed, you will be contacted by WestCycle who will provide further support and advice throughout the claim lodgement process.

How to lodge a claim

  1. Fill out an incident report, or contact WestCycle directly at info@westcycle.org.au.
  2. Complete the Chubb Injury Claim Form (notice in writing must be sent to Chubb within 30 days from the loss occurrence).
  3. Include supporting documents with the claim form such as medical reports from specialist/s and receipts for treatment.
  4. Ensure your claim form is signed by a medical specialists prior to lodgement.
  5. Lodge your claim direct to Chubb Insurance on a&hclaims.au@chubb.com.
  6. Once the claim has been registered, you will receive a claim number as a reference. Chubb can be contacted on 1300 795 779 for any questions or updates on the claim status.

Personal Accident Claim Lodgement Process

The WestCycle Inc. Member Accident Program is underwritten through Chubb Insurance and is generally a reimbursement policy meaning the member must pay for treatment to then be reimbursed.
  1. Pre Lodgement Enquires – please contact either WestCycle or Tresidder Insurance Group on 03 9021 7700 for any questions prior to the claim lodgemen.
  2. Please complete the Chubb Injury Claim Form Membership is required to be validated prior to acceptance of the claim.
  3. All validation requests are required to be treated with priority to avoid any holdup.
  4. Notice in writing must be sent to Chubb within 30 days from the loss occurrence.
  5. Please include supporting documents with the claim form such as medical reports from a specialist and receipts for treatment.
  6. Your Claim form will need to be signed by a medical specialist prior to lodgement.
  7. Please lodge your claim direct to Chubb Insurance on a&hclaims.au@chubb.com.

Tim Roach (Chair)

Tim has been in senior leadership and strategic development roles for more than 20 years and is currently a project risk consultant, and a part time academic at Edith Cowan University.

He is a past Director of Executive Education in the School of Business and Law at Edith Cowan University and Assistant Commissioner and General Manager in the public service. He is an Accountant (FCPA) and previously sat on the Divisional Council of CPA Australia. Tim has been involved in racing mountain bikes, BMX and triathlon for many years, both as a father of two children who race and as a past and current bike racer.

Helen Sadler (Deputy Chair)

Independent Director

Helen is a Town of Cottesloe councillor and is the current Chair of WestCycle’s Transport Advisory Group.

A medical doctor, Helen is a strong active transport advocate with a focus on health outcomes and social well-being.
